I went back to check on his older videos, and I’m pretty sure Tom has had those ear-plugs for some time, only they were removed (or he had transparent ones) and the long hair always covered the lobes. I think he’s just come out of the closet, as it were.

Small plugs can be nice, but when they get so big the lobe loses blood-flow and finally dies off, that’s pretty gross.
